For Primary Care:

Medicare, Medicaid, GNOCHC and commercial insurance for covered services are accepted. Sliding scale fee based on income and household size available for persons whose household income is between 100% and 200% of Federal Poverty Guidelines, minimum fee of $25.   Fees for laboratory tests and other procedures are at additional cost. 
Bring proof of household income and picture identification card.

For Dental:
Medicare, Medicaid, and commercial insurance for covered services are accepted. Sliding scale fee based on income and household size available in compliance with Federal Poverty Guidelines. Bring proof of household income and picture identification card.

For Behavioral Health:

​Medicare, Medicaid, GNOCHC and commercial insurance for covered services are accepted. Sliding scale fee based on income and household size available for persons whose household income is between 100% and 200% of Federal Poverty Guidelines, minimum fee of $25.   Fees for laboratory tests and other procedures are at additional cost.

-dental services (including: diagnostic, preventative, restorative, periodontal, oral surgery, and other adjunctive dental services)

-behavioral health (including: individual and family therapy and psychiatric assessments)

-mobile dental unit visits to various locations and organizations
