• Chronic Illness Management: Asthma,

Cardiovascular, Diabetes

 

Medical care and treatment to improve health

outcomes for persons with chronic diseases, such

as asthma, high blood pressure, hepatitis C,

diabetes, etc. Chronic disease care includes care

coordination, patient and family education, and

supportive services to remove barriers to care.

 

• Family Planning

 

 Medical services related to birth control,

including gynecological examinations, medication,

and supplies. Family planning may be provided by a physician or nurse practitioner.

• Immunizations

 

 Vaccinations to protect against specific

preventable diseases according to recommended schedules by age.
